Crisis Intervention Services

What is Crisis Intervention?

  • Crisis Intervention provides mental health intervention, assessment, support, screening, and referral services 24 hours a day, 7 days a week.

  • Services are available for anyone who calls, chats, texts, walks in, or requests a Crisis Specialist to come to their home, school, or community.

  • Follow-up support and referrals are available to other community providers if needed.

What is Crisis Intervention like?

You will be greeted by a trained Crisis Specialist in-person or over the phone.

  • The Crisis Specialist will assess the situation and determine if you are in any immediate danger.

  • If you need care, the Crisis Specialist will find a hospital or facility close to you.

  • The Crisis Specialist will provide a safe space to actively listen and create a plan for future needs.

  • The Crisis Specialist will connect you to community resources for your situation.

  • The Crisis Specialist will offer voluntary follow-up contacts to see if you are in need of any other resources or help.

All Crisis Intervention services are free and confidential.

How CCR Can Help:

Planning

  • Using a person-centered approach and team process, we help the participant and their family develop a service plan.

Services and Supports

  • Participants and their families are informed of services and supports available to them; we then assist in the selection of qualified providers.

  • Once a provider is chosen, we connect the participant with the provider via a referral. We offer continued follow-up support to all parties.

Monitoring

  • We visit with the participant and their formal and informal supports to ensure that the chosen providers and service plan meets the participant’s needs.

Community Integration

  • Through the promotion of community integration, we are able to meet the participant’s needs in the least restrictive manner.

Emergency and Incident Management

  • We respond to and assess emergency situations and incidents to ensure appropriate actions are taken to protect the participant’s health and welfare.

CCR's Supports Coordination program has been in operation for over 20 years. We assist participants and their families seeking waiver-funded services, which include:

  • Behavior Support

  • Community Participation Support

  • Employment Services

  • In-Home and Community Support

  • Respite

  • Residential Services

  • Transportation Services

We also assist families supporting participants with their general needs, which may include:

  • Food Pantries/Community Meals

  • Housing

  • Transportation Options

  • Community Activities and Events
