Housing Services

CCR provides housing service options for individuals with a mental health diagnosis from a medical provider. Individuals experiencing homelessness or at risk of becoming homeless qualify. Individuals can call CCR to complete a referral form to access services.

CCR Service Coordinators will:

  • Assess your referral and current housing needs.

  • Connect you to transitional and/or permanent housing, if needed.

  • Help you find employment, if needed.

  • Confirm any needed mental health services are in place.

  • Complete referrals for peer support or blended case management services, if needed.

Services Include:

  • Emergency Shelter

  • Permanent Supported Housing

  • Case Management

  • Life Skills Training

  • Outreach and Advocacy

  • Vocational/Educational Support

  • Participant Savings Plan

  • Transitional Housing
