What to Expect
The Care Manager will:
Greet you in-person or by phone.
Perform an intake to determine your areas of need.
Provide you with appropriate referrals and linkages to meet your identified needs.
Provide scheduled follow ups to ensure referrals and linkages were successful.
How Does the Program Work?
Provides support to individuals navigating the health and social services systems.
Conducts health and social needs screenings while educating individuals on prevention strategies.
Initiates referrals and coordinates health and community services.
Advocates for individual and community needs.
Provides direct services such as advocacy and service coordination.
Builds individuals' ability to achieve wellness and increases the community's ability to address social determinants of health.
SDOH Services are an initiative of Butler County Human Services
