CCR’s HIV+ Case Management services provide assistance in accessing resources and supports, which include:

  • Educational/Vocational Training

  • Employment

  • Transportation

  • Physical/Mental Health Needs

  • Drug & Alcohol Services

  • Legal Assistance

  • Family/Social Needs

How CCR can help:

Emergency Financial Assistance

CCR's HIV+ Case Managers will work with you to resolve problems such as utility shut-off notices or evictions. Additional support may be available for qualified needs through a one-time financial assistance request.

Support Groups

Join us for informal meetings that offer support, hope, and connection for those living with HIV+. Meetings occur 1-2 times a month within Butler County. Meeting participants share a meal and go home with leftovers.

Transportation Reimbursement

The cost of travel should not keep you from your medical or mental health appointments. Case Managers can help with accessing bus passes or mileage reimbursement for qualifying appointments.

Guided Shopping

Qualifying participants will be allotted funds to participate in guided grocery shopping. With assistance from a Case Manager, you will purchase eligible items such as meats, dairy, vegetables, grains, personal care items, and nutritional supplements.
