How CCR Can Help:
Planning
Using a person-centered approach and team process, we help the participant and their family develop a service plan.
Services and Supports
Participants and their families are informed of services and supports available to them, and we assist in the selection of qualified providers.
Once a provider is chosen, we connect the participant with the provider via a referral. We continue to offer follow-up support to all parties.
Monitoring
We visit with the participant and their formal and informal supports to ensure that the chosen providers and service plan meets the participant’s needs.
Community Integration
Through the promotion of community integration, we are able to meet the participant’s needs in the least restrictive manner.
Emergency and Incident Management
We respond to and assess emergency situations and incidents to ensure appropriate actions are taken to protect the participant’s health and welfare.

What is CCR’s MORE Program?
Our MORE Program offers caring, compassionate, and knowledgeable Case Managers to assist women who are pregnant and/or parenting (up to 18 months postpartum) and are dealing with or recovering from substance abuse. CCR’s Case Managers will work with you to develop and/or achieve your recovery goals.
How CCR’s Case Managers can help:
Coordinate primary, prenatal, and postpartum medical care with substance use services.
Assist with community and social linkages to recovery support activities such as groups, mentoring, and coaching.
Assess and monitor for mental health needs, making referrals as needed.
Assist with planning and goal setting.
Establish a positive network of supports to promote independence.
